Please understand that our office serves many patients with many different insurance policies. It is your responsibility to know the benefits of your insurance plan.
Therefore, we ask that you present your insurance card at each visit to our office. We require a copy of your insurance card on file in order to file claims. Uninsured Patients 20% discount for uninsured patients! We accept cash, and credit/debit cards. Cash and credit/debit card patients receive a 20% discount on all services except laboratory services performed by an outside lab. Insured Patients Please be aware that we accept many, but not all insurances, and you are responsible for payment if payment is refused by your insurance company! Click Here to see if your insurance company is one we accept. Co-payments are expected at time of service. Workers Compensation Patients Prior authorization for office visits and procedures is required. Injured workers are not charged for authorized services. Payment in cash, check, money orders, or major credit card is requested at the time of service, and all co-payments and deductibles will be collected. Please be advised that there may be services your insurance company will not cover (new immunizations, flu shots, vision or hearing tests, etc.). We request full payment for these services at the time rendered.

As a courtesy to our patients with insurance coverage, our staff will file your claim if you provide the current insurance information. Although an insurance claim is filed on the patient’s behalf, the financial responsibility for our services still remains with the patient and not their insurance company. If we do not receive payment within 45 days, it becomes the responsibility of the patient to contact the insurance company to expedite claims processing.
